Confidentiality
Information shared during sessions is kept confidential and will only be released with your permission, except in circumstances required by law or professional ethics: imminent risk of harm to self or others; concerns regarding minor children (18 years or under) or dependent adults and their well-being; or if counselling information is subpoenaed by a court of law.
Your name and telephone number may be stored electronically on a web-based application. On occasion, insurance companies may contact Leanne for confirmation of session dates and attendance after you submit invoices for reimbursement. By providing your initials in the field above, you agree to Leanne Perrin releasing that information.
Counselling Relationship
Leanne emphasizes the importance of working together as a team and recommends clients engage in self-help between sessions to maximize the effectiveness of therapy. She provides health and wellness skills, tools, and information to support the therapy process.
Your Rights
Clients have the right to ask questions about treatment, make informed decisions about the methods used, and stop therapy if it is not working for them. Leanne encourages feedback and may suggest alternate resources if therapy needs to be discontinued.
Risks of Psychotherapy
Psychotherapy involves a degree of risk, including experiencing uncomfortable emotions as you talk about your history. Any change can affect a person's established system. There is often dysregulation and even identity reconstruction as your system integrates new adaptive information and replaces the old maladaptive information, such as that accessed through EMDR therapy.
Fees & Session Length
Leanne's current rate is $200.00 per clinical hour (60 minutes of session time). Depending on EAP coverage or other circumstances, a sliding scale may be applied. An invoice is sent following the appointment and payment may be made by e-transfer. EMDR Therapy sessions are usually 90–120 minutes. Separate fees may apply for letters/reports and Intensive Therapy.
Appointments
Clients are seen by prearranged appointment. Telehealth links are sent to your email. Telehealth platforms are PIPEDA/PHIPA Compliant. Telehealth hours are primarily 8:00 A.M.–5:00 P.M. Monday to Friday, with exceptions case by case. If urgent, call 811. In an emergency, call 911 or visit your local hospital.
Cancellation & No-Show Policy
Please cancel or reschedule 24 hours in advance to avoid a late cancellation fee. Late cancellations and no-shows are billed at the full hour rate. Outstanding accounts incur a 2% penalty if not paid within 30 days.
By signing above I agree to e-transfer the amount due upon receiving an invoice from Leanne Perrin following my appointment, late cancellation, or no-show, and to pay a 2% penalty if delayed by more than 30 days.
